If you experience an on the job injury it is extremely important to complete a First Report of Injury Form.  This form must be filled out, printed, and signed.  You will send the completed FROI form to Misty Ratliff and to your medical provider if you seek medical attention.  The First Fill Form is to be filled in and given to the pharmacy if your physician prescribes medication for your injury. The Worker’s Comp Claim Information list helpful names, phone numbers, and addresses.
