The following are forms that are required by the district to be filled out and signed by the students parent or guardian , as well as PHYSCIANS signature on all Action Plans and Inhalers that student will carry on their persons. These forms must be filled out and returned to the campus nurse in order for the nursing department to administer over the counter or prescription medications.
