Field Trip Request -High School

High School - Field Trip Request

When submitted, this form will digitally be sent to the high school principal and/or assistant principal, for consideration.  
Form should be submitted at least two (2) weeks prior to date of trip/activity.


A list of all students, teachers, and chaperones participating in trip/activity must be submitted with Field Trip Final Review Form to campus principal five(5) days prior to trip/activity.
